ONLINE REGISTRATION
WELCOME TO OUR ONLINE REGISTRATION PROCESS. PLEASE READ THROUGH THE ENTIRE PAGE PRIOR TO BEGINNING THE ONLINE APPLICATION.
IN ORDER TO COMPLETE ONLINE REGISTRATION, HARD COPIES OF CERTAIN DOCUMENTS AND PROOF OF RESIDENCY WILL BE REQUIRED TO BE UPLOADED DURING THE PROCESS.
Please print and complete the following forms for each student PRIOR to beginning online registration. If you have any questions, contact us at the above number.
ALL REQUIRED FORMS WILL NEED TO BE SCANNED AND UPLOADING DURING THE PROCESS. PDF or photo formats (TIFF, JPEG, PNG, GIF) will be acceptable.

OBTAINED FROM PRIMARY CARE PHYSICIAN (MUST BE UPLOADED FOR ALL NEW STUDENTS) - Dental Health Certificate
(MUST BE UPLOADED FOR ALL NEW STUDENTS) - Records Release Form ONLY FOR TRANSFER STUDENTS
(TO BE SENT TO PREVIOUS SCHOOL DISTRICT) - Housing Questionnaire

If there are any custodial issues regarding this student, you will need to print and UPLOAD one of the following:
- Parent Affidavit - to be completed by Parent (MUST BE NOTARIZED)
- Custodial Affidavit - to be completed by Custodian (MUST BE NOTARIZED)
- Guardian Affidavit - Request by a Non-Parent that pupil residing with him be considered a resident of Wantagh Public Schools
If you do not own your home, you MUST print and upload one of the following:
- Landlord Affidavit (MUST BE NOTARIZED)
- Tenant Affidavit (MUST BE NOTARIZED)
*If you are unable to complete your registration online, please reach out to Central Registration at (516) 765-4165 or at centralregistration@wantagschools.org to make an appointment to register in person. For in person registration please go to the "Registration Forms" page to print out and complete the necessary forms. Review the Registration Requirements below. Acquire necessary notarization and make copies of required supporting documents (tax receipts, immunization records, etc.)
*Note: Enrollment may be denied if complete documentation is not received by the Registration Office within 3 business days following the registration appointment.

Registration Requirements
Children residing with parents or a legal guardian within the district are eligible to attend the Wantagh Public Schools. Children who are residents will be admitted to school provided that they:
- are five years of age on or before December 1st of the year of entrance to Kindergarten (birth certificate required)
- have proof of all required immunizations
- have proof of residency (2 proofs required, see below for examples)
Acceptable Forms of Documentation
- Original, embossed birth certificate or passport
- Certificate of Immunization
- Proof of Residency
- Parent, Guardian and Custodial Stipulation Forms, if applicable, accompanied by duly executed court documents indicating legal guardianship and/or custody
If you Own Your Home
Current Mortgage Statement
OR
Mortgage Closing Documents or Deed
OR
Town of Hempstead Tax Receipt
If you Rent or Lease Your Home
Notarized Rental/Lease Agreement or Notarized Landlord Affidavit
AND
Landlord's copy of Town of Hempstead Tax Receipt
If you Live with a Family Member who Owns the Home
Notarized Rental/Lease Agreement or Notarized Landlord Affidavit
AND
Notarized Affidavit to Substantiate Residency
AND
Landlord's copy of Town of Hempstead Tax Receipt
Plus, any ONE of the following:
- Driver's license of parent/guardian
- Utility bill of residence, i.e. electric, oil, cable, phone, etc.
- Printed name and address label issued by the Internal Revenue Service
- Copy of forwarding address card that is post marked from the post office
- Updated voter's registration card
**Acceptance of other appropriate proofs shall be at the discretion of the Wantagh School District.
*Please provide any custody papers at the time of registration and let us know if there are any issues regarding the supervision of your child.
