If you are injured at work it is important to let us know immediately.

• If you are in need of an ambulance or will bring yourself to the doctor or emergency room, please tell someone before you leave.
• When you arrive at the facility, please let them know that this is a Worker's Comp claim and that our insurance is with Wright Risk Insurance Group.
• Once you have been set up, Wright Risk will issue a claim number to you and the providers can use that number.
• You must complete C-3 and C-3.3 forms and return them to Human Resources as soon as possible.  This is how a claim number is formed.
